1. Record what happened

Three dated observations from Sophia's classroom.

  1. Sophia joined each planned activity after the routine reset, with no reminder noted today. The current routine appears to be supporting participation.
  2. After the transition, Sophia rejoined group work with one prompt and stayed with the activity. The team can keep watching how transitions go.
  3. After a two-minute warning before cleanup, Sophia moved into the next activity without another prompt. The teacher plans to continue this support.
2. Choose the evidence and add context

The teacher's meeting summary

Sophia's selected notes describe participation, transition support and classroom routines. The family and team can review these dated observations before deciding what support to continue.

This sample includes all three notes above. The teacher selects what belongs in the packet and writes the summary.
