ABC Data Sheet

A free printable ABC data sheet for recording antecedent, behavior, and consequence during an FBA, with a column for what the student got or avoided. Free to print, copy, and hand to a paraeducator. No account, no email address.

Use this sheet when the concern sounds like

  • “leaves the room without permission a few times a week, and nobody is sure why”
  • “calls out during whole-group lessons, and redirecting has not changed it”
  • “the team is starting an FBA and needs direct observation data”

ABC data is the direct-observation part of a functional behavior assessment. One sheet rarely settles anything; the useful moment comes when the same antecedent and the same consequence keep showing up across different days, settings, and adults. Keep each sheet to one student and one clearly defined behavior, and read the rows after each session instead of saving them for the meeting, so the team can see when the pattern settles. Calm stretches in the same activity are worth a row too: a note that the behavior did not happen tells the team as much as one that says it did.

Got / avoided: AA = adult attention · PA = peer attention · E = escaped or delayed a task · IT = got an item or activity · S = sensory or automatic · ? = unclear

  1. One row per occurrence, written as soon after it as you can. Describe what a camera would have recorded: "pushed the worksheet off the desk", not "got frustrated".
  2. Antecedent is what happened in the moments right before: a direction, a transition, an adult turning away. Consequence is what happened right after, including what adults and peers did.
  3. The last column is what the student ended up with or out of, not a guess at the reason. The team decides function later, from the pattern across many rows.
