Yes/no is the fastest to record and the easiest to over-summarize. Pairing every entry with a prompt level is what keeps it honest, and it is what lets a flat-looking goal show real progress as support fades.

Prompt key: I = independent · V = verbal · G = gestural · M = model · PP = partial physical · FP = full physical
- One line per opportunity, not per day. A day summarized as one yes hides the three opportunities that went differently.
- Prompt level is what turns a yes into useful data. "Yes with full physical support" and "yes independently" are different results.
- Use the notes column for the thing you would have told a colleague at lunch. That is usually the sentence you will want at the meeting.
