Prompt Level Data Sheet

A free printable prompt level data sheet for tracking how much support a student needed on each trial, and whether that support is fading over time. Free to print, copy, and hand to a paraeducator. No account, no email address.

Use this sheet when the goal reads like

  • “will complete the hand-washing routine with no more than 1 verbal prompt”
  • “will request a break independently, fading from a gestural prompt”
  • “will begin independent work after a single verbal direction”

Most goals get scored correct or incorrect, which makes a student look stuck while the real change is happening underneath: the same step, with less help. Reading down the last column is usually the fastest way to show a family or a team that a flat-looking goal is moving. If your goal is written with a target prompt level in it, this sheet is the one that matches the wording.

Prompt key: I = independent · V = verbal · G = gestural · M = model · PP = partial physical · FP = full physical

  1. Write the prompt code in each trial box rather than a tick. The code is the data; a tick only tells you the step happened.
  2. Fill the last column with the least intrusive prompt the student succeeded with that day. That single column is the progress line.
  3. Leave a box blank if the opportunity did not come up. A blank is honest; a full physical you did not actually give is not.
