A one-page template a teacher fills out so a substitute or covering colleague can keep behavior logging going during an absence. The aim is no lost days: a sub should be able to log the right goals at the right moments without needing the regular teacher's full context.
List the students and their one logging moment
Name each student on the caseload and the single moment their chart gets logged, such as after the morning block or during dismissal. Keep it to the logging moment that already exists, not an ideal one the sub cannot run. A short, accurate list beats a thorough one a covering teacher will not have time to read.
State each goal in plain, observable terms
For every student, write the one to three goals in language a sub can score without knowing the backstory: begins work within two minutes, stays in assigned area, uses a break card. Avoid jargon and IEP shorthand. If the regular teacher would have to explain a term, rewrite it so the sub does not have to ask.
Note the essential supports and triggers
Flag the few supports that must continue and the triggers worth knowing, like a transition warning that prevents elopement or a calm corner the student is allowed to use. Keep this to what affects safety and logging, not the entire plan. A covering teacher can hold three or four critical notes, not thirty.
Explain how and when to log
Tell the sub exactly where to log and what to do if they are unsure: leave an honest gap rather than guess, and add a short note if something unusual happened. Logging access can be coordinated where permissions allow, so a covering teacher on the same students is not locked out. An honest gap with a note is far better than invented data the regular teacher has to untangle.
Leave a contact and a back-to-normal note
Add who to reach for a real question and a line on anything time-sensitive, like a meeting or a family contact expected that day. When the regular teacher returns, a quick scan of the sub's notes and any gaps is enough to pick the routine back up. The one-pager is a bridge across the absence, not a full plan rewrite.
